MEDICAL RELEASE FORM

You should always be prepared for medical emergencies in your group. We recommend that you have each youth/parent fill out a form and sign. This form should include the following information:

Youth's full name
Insurance company & policy number
Allergies
Date of last tetanus shot
Emergency phone number
Authorization of youth leaders to sign for minors in case of emergency (parent's signature)
