[Snail mail]: email us your name and address to request a packet.

[Electronic Forms]: download the application packet in PDF and Microsoft Word format, complete the forms electronically and email them to RMM. (600 Kb)

Download instructions: Both packets are compressed folders containing all of the files that you need to initiate the application process. Click on the appropriate link and choose "Save." After the file has been saved to your hard drive open the folder, extract the files, and open the "Contents" file for instructions. If you do not have Adobe Acrobat Reader® you can download it free-of-charge.
